What does a nurse navigator do?
Health and well-being are complex, understanding and dealing with care providers, health providers and coordinating it all can be quiet overwhelming.
A Nurse Navigator is a highly experienced registered nurse with a wealth of knowledge about health and care systems, this means we can work with you, to find out your individual needs and wants when it comes to your health and well-being journey. We believe that home is the best place for you to be cared for when at all possible. We are your advocate, working with you to find your choice of care.
We can help educate you to lead the way, empowering you with the knowledge needed to access the care you need.
We support you to access services, ensuring that your needs are met. We can be the person you call when you’re not sure what do next, when those unexpected things happen the reassurance of having a professional to contact.
Encompass Nurse Navigator Service is person centered with you as our client at the forefront of what we do. We recognise that everyone has individual unique requirements. Our caring Nurse Navigators will listen to your needs and offer choices to help you achieve them. Our aim is making your life easier so your quality of life is the best to can be.
We can help you look at options whether that be government funded or Privately funded, to achieve that best care for your needs.
We will support you to navigate difficult topics and complex issues compassionately. Help organise the highest quality care possible for your specific needs and advocate on your behalf for all your health needs. You can be reassured that our nurses have your best interests at heart and can be with you every step of the way.

At Encompass Nurse Navigator Service we provide a holistic patient centered experience. From the moment you refer to us you can be confident your patients are in good hands. As an independent service our highly experienced nurses provide a comprehensive person-centered assessment to establish the patient’s needs, working collaboratively with them, their families and you as their medical team to ensure they receive the care and support they require.

How are they going to find the care they need, someone to support them to complete that investigation you just requested so you can move forward with their medical care?
This is where Encompass Nurse Navigators can help.
As depicted in the diagram below, we wrap around the person supporting every aspect of their needs. Firstly, assessing their current needs, working together to access the services they need to live their best lives, secondly planning for the future.
We work with the person and their families to support them to access community resources, Allied health assistance, gain approvals for home care packages, access privately funded care, approvals for accessing respite care and / or Nursing home placement to name a few outcomes.

What you can expect when you refer to us:
Communication is key!
– Confirmation of receipt of referral within 24 hours
– Contact will be made with the patient or contact person within 48 hours of receipt of referral to arrange assessment
– Assessment visit will be booked at a time suitable to the patient / carer.
– Our attendance at your practice for case conferencing, should you feel appropriate (Medicare item number 735)
– Comprehensive letter for your records informing you of the outcome of initial assessment and plan of care
– Regular clear communication updating you of our achievements
